RAC Tracking Software, RAC Preparation Audits and Denial Management

LexiCode will assist you in identifying and correcting improper Medicare payments due to coding, documentation and medical necessity issues. We conduct focused reviews of those high risk areas historically targeted in RAC audits and provide you with the information you need to be prepared and to formulate corrective action measures. LexiCode can perform an onsite or remote review of medical records for the purposes of validating compliance with Medicare.

LexiCode can also assist with the medical record review of RAC denials and RAC appeal preparation.

• Skilled, credentialed members of the LexiCode team will evaluate the RAC determination, along with documentation in the medical record to determine the appropriateness of utilizing the discussion period and/or initiating the formal appeal process.
• LexiCode will prepare the written rebuttals and/or appeals, including submissions for redetermination and requests for reconsideration at various appeal levels, in accordance with appropriate timelines.

Once the RAC has notified you of an audit, LexiCode will utilize our RAC tracking software, SOURCECORP RACSource to:

• Track all RAC requests;
• Track the timeline for rebuttal and appeals;
• Assign tasks to the appropriate parties involved in the appeal process;
• Collect data to analyze trends with RAC requests;
• Report financial impact of RAC take backs;
• Store all documents necessary for managing the RAC process including the medical record, appeal letters, & UB-04.

SOURCECORP RACSource is a sophisticated, customizable tracking and reporting tool that will assist your facility not only with responding to RAC requests but with being pro-active by identifying target areas for improving processes and providing education programs for coding, billing, case management and physician documentation to prevent future RAC take backs and insure optimal reimbursement.
